T&Cs for Ultraview Readers 'Win Free Treatment' competition
This offer is open to UK optometrists and contact lens opticians who visit www.ultralase.com/ultraeyecare and enter their name, address and contact information.
- The Ultralase Ultraview Prize Draw will take place on Friday 8th January 2010.
- The closing date for entries will be Wednesday 6th January 2010.
- The winner is entitled to free laser eye surgery, worth up to £4,500 (£2250 per eye).
- Treatment is subject to a free consultation at which suitability is assessed and the most appropriate vision correction treatment type recommended.
- Even though Ultralase has such a wide range of vision correction treatments, please note that around 10% of those attending consultations are deemed unsuitable for treatment.
- If the prize winner is not deemed suitable for vision correction treatment, the prize may be transferred to a practice colleague. Ultralase will require proof of employment if this prize is transferred to a practice colleague.
- The surgeon's decision whether to proceed is final.
- There is no cash alternative.
- Those who have already had vision correction treatment will not be entered into the draw.
- Open only to UK residents.
- Entrants must be over 18 years of age.
- The winner will be notified by telephone and / or post by Friday 18th December 2009.
- The winner will be required to provide a photograph, testimonial and answer questions about their treatment which Ultralase will be permitted to publish in marketing material and optical or business press.
